Coffee Bean Sign Radiology. The “coffee bean” sign is seen on supine plain films of the abdomen in approximately 80% of cases of sigmoid volvulus. The sigmoid colon is very dilated because it is twisted at the root of its mesentery in the left iliac fossa (lif).
